Explanation: Caihu /Bupleurum root, as a principle herb, has the effects of dispelling pathogenic factor located in the half exterior of Shaoyang meridian by driving it out; it can also relieve the depressed liver-qi. Huangqin / Scutellaria root clears out stagnated heat located in the half interior by clearing away it thoroughly as an assistant herb. The combination of 2 herbs, one for dispelling pathogen out; and the other for clearing away stagnated heat from the interior, together can remove pathogenic factors from Shaoyang. Banxia/Pinellia tuber and Shengjiang / Fresh ginger can regulate the function of the stomach and lower the adverse flow of qi. Renshen /Ginseng and Da Zao /Chinese dates invigorate qi and strengthen the middle-jiao. Zhigancao/Prepared licorice root can coordinate the actions of various herbs in the recipe. All the above can act as adjuvant and guiding herbs.

Indications in the original texts of Shanghan Lun

Clause 96: After 5-6 days during febrile disease, the patient has alternating attacks of chills and fever, feels a distension and a sensation of oppression in the chest and costal region, reluctant to speak and eat, restlessness and no nauseous, or thirsty, or abdominal pain, or with mass below the costal margin, or with palpitation and dysuria, or no thirsty but with a slight fever, or with coughing. Xiao Caihu Tang /Minor Bupleurum Decoction can serve as a remedy in this case.

Note: Key indications of Xiao Caihu Tang/ Minor Bupleurum Decoction:

1)     Alternate chills and fever;

2)    Feeling a distension and a feeling of oppression in the chest and costal region;

3)    Reluctant to eat and speak;

4)     Restlessness and nausea.

There are other clauses on Xiao Caihu Tang /Minor Bupleurum Decoction, such as 37, 97, 98, 99,100, 101,103, 104, 144, 148,149, 266, 229, 230, 231, 379, 394 in Shanghan Lun.

Modern clinical application

1)      Clinically, we often use Xiao Caihu Tang / Minor Bupleurum Decoction for the treatments of common cold, malaria, infection of Biliary tract, hepatitis, pleurisy, chronic gastritis, indigestion, mastosis, inter costal neuralgia, neurosis and AIDS marked by the symptoms of Shaoyang disease. Of course, we often modify the recipe according to each special different condition 

2)      Prof. Liu Duzhou introduced that the recipe is effective for impotence caused by stagnation of the liver-qi. See the book, ¡°Research in Classical Prescriptions of TCM¡±.

3)      Dr Li Xinghua reported this recipe can be used for acute pancreatitis.

Modern research

1)     Modern researches have proved that the recipe has some effects of inhibiting bacteria, viruses and leptospira, relieving the reaction of the human body to the invaded pathogen, and remarkably allaying fever and resisting inflammation. 2)     The recipe also has the effects of promoting digestion, preventing vomiting, expelling phlegm, relieve cough, protecting the liver, normalizing the function of gallbladder and tranquilizing the mind. See the same source. From the same source.

(3)  Taiyin (Great yin) Disease: It is an advanced stage and condition that can be caused by mistreatment of 3 yang meridians diseases, especially the Yangming Disease evolves Taiyin Disease, marked by deficiency of the spleen-yang due to invasion by pathogenic cold-dampness, manifested as abdominal distension, poor appetite, nausea or vomiting, diarrhoea with loose stools, and occasional abdominal pain, its therapeutic principle: Warming the spleen-yang; Recipe: Sini Tang / Decoction for Resuscitation or its like ( Lizhong Wan /Pill  for Regulating Middle-jiao can be adopted. Note: a purgative is prohibited to this case.

Chief   Recipe in Taiyin Disease:

Lizhong Wan/ Pill for Regulating Middle-jiao

Ingredients:

Renshen/ Ginseng/ Radix Ginseng, 3 Liang/9g

Ganjiang/Dried ginger/ Rhizoma Zingiberis, 3 Liang/ 9g

Zhigancao/Prepared licorice root/ Radix Glycyrrhizae Praeparatae 3 Liang/9g

Baizhu/White atractylodes rhizome / Rhizoma Atractylodis Macrocephalae   3 Liang 9g

Administration: According to the original texts of Shanghan Lun and our experience, we should first grind the above herbs into the fine powder, and then mix them with honey to make them into pills. Take the pills with warm boiled water, 9g each time, 3-4 times on a day, 2 times a night if necessary; or decoct the herbs in water in the pot for 25-30 minutes, and then get the decoction which will be taken orally in 3 separate times in a day.

Actions: Warming the middle-jiao and eliminating pathogenic cold-dampness from Taiyin.

Explanation: Ganjiang/Dried ginger, acts as a principle herb, acrid in taste and hot in nature, has the effects of warming the middle-jiao; Renshen/Ginseng acts an assistant herb, slightly bitter in flavour and slightly warm in nature, and    is good in strengthening the spleen-qi; Baizhu / White atractylodes rhizome, serves as an adjuvant herb, with its sweet and bitter flavour and warm nature, and  is used to dry dampness and strengthen the spleen; Zhigancao/Prepared licorice root, sweet and mild in property, harmonizes the spleen and stomach and tempers the effects of other herbs in the recipe, playing a guiding herb.

Indications in the original texts of Shanghan Lun

Clause 277: Diarrhoea without a thirsty for water is the symptom of Taiyin disease. It is caused by coldness in the viscus. Sini Tang/ Decoction for Resuscitation or its like can be adopted to warm the coldness.

Notes:

1)     As the above case is in a deficient and cold syndrome , so the patient feels no thirsty;

2)     The viscus here means the spleen in this clause.

3)     ¡°Sini Tang or its like¡± means the Lizhong Tang/ or Lizhong Wan.

There are other clauses such as 386, 396 in Shanghan Lun; also seen in Jin Kui Yao Lue in Chapter 9.

Modern clinical application

1)   We often use Lizhong Tang/ Decoction for Regulating Middle-jiao for the treatments of acute or chronic gastritis, peptic ulcer, gastro-intestinal neurosis, chronic diarrhoea, chronic enteritis or non-specific colitis, and AIDS, etc. It will be seen in ¡°New Research in Classical Prescriptions of TCM¡±.

2)     Therapy on the umbilicus: Dr Li Zhongjie introduced to make an ointment of Lizhong Wan / Bolus for Regulating Middle-jiao and then apply it on the umbilicus; cover it with compress and keep it for 3-7 days which can be used   to treat chronic diarrhoea, edema, cough, and uterus bleeding. For more details, please see the book ¡®Research in Classical Formulae of TCM¡¯.

Modern research

1)       The Pharmaceutical Dept and Microbiology Dept of Shanghai TCM University made a simulated syndrome of spleen-deficiency in a mouse by feeding Dahuang/ Rhubarb /Radix et Rhizoma Rhei, as a result the mouse   shown symptoms of spleen-deficiency marked by diarrhoea as well as lower function of immune system and less ability against coldness; then they treated the mouse with Lizhong Tang/Decoction for Regulating Middle-jiao. After that the mouse¡¯s   symptoms were relieved. See the book, ¡°Research in Classical Prescriptions of TCM¡±.

2)       Many new studies have also ascertained that this recipe has the effects in adjusting the functions of the stomach and intestine, promoting blood circulation, strengthening metabolism, relieve spasm, alleviating pain, arresting vomiting and diarrhoea, and inducing diuresis. Shaoyin (Lesser yin) Disease: This is a more severe stage and condition during febrile diseases. In TCM theory, Shaoyin includes the kidney meridian of foot Shaoyin and the heart meridian of hand Shaoyin. The causes of Shaoyin Disease include many factors: exterior pathogens    intrude interior; or Taiyin disease develop to Shaoyin disease; pathogenic cold invades Shaoyin meridian directly; or both Taiyang and Shaoyin are affected by pathogenic factors.

Shaoyin disease is divided into 2 main syndromes:

First, Cold syndrome, manifested as Clause 281 ¡®the essential symptoms and signs of Shaoyin Disease are a feeble and thready pulse, a tendency for the patient to continue fall asleep¡¯ , its therapeutic principle is described in Clause 323 ¡®therapeutic method of warming, Sini  Tang/Decoction for Resuscitation should be given urgently to Shaoyin disease with a deep pulse;

Second, Heat syndrome, marked by yin deficiency with yang hyperactivity, manifested as in Clause 303 ¡® more than 2 or 3 days of Shaoyin disease , the patient feels restless and suffers insomnia, Huanglian Ejiao Tang/Coptis and Donkey-hide Gelatin Decoction will act as a curative in the case.

Chief Recipes in Shaoyin Disease

Sini Tang/ Decoction for Resuscitation

Ingredients:

Fuzi/Aconite root/ Radix Aconiti Praeparatae   1 pcs/ 9g

Ganjiang/Dried ginger/ Rhizoma Zingiberis 1.5 Liang/9g

Zhigancao/ Prepared licorice root / Radix Glycyrrhizae Praeparatae 2 Liang/ 6 g

Administration: According to our experience,  Fuzi /Aconite root should be decocted one hour prior to other two; and then add other 2 ingredients and boil 3 ingredients again for 45minutes or more. Get the decoction for oral use when it is warm. Drink half in the morning and the other half in the evening.

Actions: Warming the heart, spleen and kidney, recuperating depleted yang to rescue the patient from collapse.

Explanation: Fuzi /Aconite root  in the recipe, pungent and hot in the extreme, acts on the meridians of the heart, kidney and spleen, helping to warm the yang-qi and recuperate the depleted yang to rescue the patient from collapse, it is preferred as a principle herb. However, in the UK, we may use Renshen/Ginseng 9g plus Huangqi/ Astragalus root 30g instead of Fuzi/Aconite root because its use is prohibited in the UK. Ganjiang /Dried ginger, an assistant herb in the recipe, functions in warming yang of the middle-jiao and eliminating interior coldness. The combination of Fuzi/Aconite root and Ganjiang /Dried ginger can develop drastic flavour to strengthen forcefully the effects of warming yang. Zhigancao/Prepared licorice, with the effects of strengthening the spleen and replenishing qi, as an adjuvant and guiding herb, is used in assisting Fuzi /Aconite root to recuperate depleted yang and promote blood circulation, and also in assisting Ganjiang /Dried ginger in warming and restoring yang of the middle-jiao to reduce toxin of Fuzi /Aconite root and relieving the extremely pungent property of other 2 herbs so as to make it possible to warm yang and remove cold without injuring yin.

Remarks: As Shengfuzi/crude Aconite contains Wutoujian (aconine) which is highly toxic and should be boiled for more than one ¨Ctwo hours until no numbness of the mouth and tongue is felt. Today Zhifuzi/Prepared Aconite is still used in TCM practice in the world except the UK, but it should be boiled for a longer time(1-2 hours), because the long time boiling can change its Wutoujian/aconine into Wutoucijia (sub-aconine) which has just 1-2 % of Wutoujian¡¯ toxin with the same efficacy. Additionally, when we meet patients with aconite poisoning, we should give patient Atropine injection and other supporting therapies on time to save our patients.

Indications in the original texts of Shanghan Lun

Clause 323: Therapeutic method of warming, Sini Tang/Decoction for Resuscitation should be given urgently to Shaoyin disease with deep pulse.

Notes:

1)     This clause is very simple .Only a pulse , and other  symptoms and signs are omitted;

2)     The pulse is deep, feeble and thready;

3)     According to other clauses on Sini Tang/ Decoction for Resuscitation, there are other symptoms and signs such as cold limbs, a feeling of chill, huddling up with cold, water diarrhoea with undigested food in the stool, mental fatigue and sleepiness, and deep, feeble and thready pulse.

4)     Other clauses on Sini Tang /Decoction for Resuscitation are clause 29, 91, 92, 225, 277, 323, 324, 353, 354, 372, 377, 388,389 in Shanghan Lun.

Modern clinical application

1)     We often used Sini Tang /Decoction for Resuscitation for the treatments of distinct declination of bodily strength caused by chronic disorders or hypopituitarism, hypothyroidism and hypoadreenocorticism marked by symptoms of yang-deficiency. 2)     We also tried to use Sini Tang /Decoction for Resuscitation for yang depletion syndrome such as shock, heart failure, cardiac infarction, etc. We also made a Sini Tang Injection many years ago, it was effective. It will be seen in ¡°New Research in Classical Prescriptions of TCM¡±.

Modern research

1)     Recent studies have confirmed that this recipe has the functions in exciting central nervous system, promoting metabolism, improving weak conditions, preventing shock. See ¡°A Practical English-Chinese Library of TCM¡±.

2)     Dr Han Xinmin made a simulated hypotension in a rabbit by injecting anesthetic to the rabbit for studying the functions of each single herb in the Sini Tang /Decoction for Resuscitation as well as the compound decoction of Sini Tang. They found a single Fuzi/ Aconite root has a cordial function, but it is weaker than compound decoction of Sini Tang; a single Zhigancao /Prepared licorice root can increase blood pressure but can not strengthen heart¡¯s contraction at all; a single Ganjiang/Dried ginger has nothing on blood pressure and heart¡¯s functions as well. When giving compound decoction of the Sini Tang to the rabbit, it shown strong cordial function, also can slowdown sinus rhythm as well as preventing arrhythmia caused by single Fuzi /Aconite root. This test has proved the synthesis of compound prescriptions in TCM .See ¡°Research in Classical Formulae of TCM¡±.

Jueyin (Absolute yin) Disease: It is a most complicated stage and condition during febrile disease. In TCM theory, Jueyin meridians include the Liver Meridian of Foot-jueyin, and Pericardium Meridian of Hand-jueyin. But in the Chapter Jueyin Disease, it mainly discuss about the disorder of Liver Meridian of Foot-Jueyin. There are 4 main syndromes in Jueyin Disease, namely: the upper-heat and lower-cold syndrome; Cold syndrome; Heat syndrome; and Preponderance between cold and heat.

1)     Upper-heat and lower-cold syndrome: It includes Wumei Wan /Black Plum Pill syndrome; Ganjiang Huangqin Huanglian Renshen Tang/ Decoction of Dried Ginger Scutellaria Coptis and Ginseng syndrome, Mahuang Shengma Tang /Decoction of Ephedra and Cimicifuge syndrome, etc.

2)     Cold syndrome includes Sini Tang syndrome (Clause 353,354), Danggui Sini   Tang syndrome (Clause 351); Danggui Sini Jia Wuzhuyu Shengjiang Tang Syndrome (Clause 352), etc.

3)     Heat syndrome of Jueyin disease includes Baitouweng Tang syndrome (Clause 373), Xiao Chengqi Tang syndrome (374), etc.

4)     The preponderance between cold and heat:  such as Clause 331 ¡°When fever occurs after a prevalence of coldness on the extremities, diarrhoea will come to a stop¡±.

Chief Recipe in Jueyin Disease

Wumei Wan/Black Plum Pill

Ingredients:

Wumei/ Black plum /Fructus Mume, 300 pcs/15g

Xixin/ Asarum herb, 6 Liang/3 g

Ganjiang/Dried ginger, 10 Liang/ 6g

Huanglian/Coptis root, 16 Liang/9g

Zhifuzi /Prepared aconite root, 6 Liang/6g

Danggui/ Chinese angelica root, 4 Liang/9g

Huajiao /Prickly ¨Cash peel, 4 Liang/10g

Guizhi/Cinnamon twig/Ramulus Cinnamomi, 6 Liang/9g

Renshen/ Ginseng/Radix Ginseng, 6 Liang/6g

Huangbai/Phellodendron bark/Cortex Phellodendri, 6 Liang/9g

Administration: According to the original texts of Shanghan Lun and our own experience, we should soak Wumei /Black plum with vinegar one night; grind the above herbs into powder to make pills, 8 pills each time, 3 time a day, then you may gradually increase its dosage according to the condition; or use common dosage of the above herbs to make a decoction for oral use, take half in the morning and another half in the evening.

Actions: Warm the interior-coldness, clearing away stomach-heat, and calming and removing the ascaris.

Explanation: The chief herb in the recipe is Wumei /Black plum that is soaked with vinegar again during processing, becomes a very sour substance which is quite effective in eliminating roundworms. The deputies are divided into 2 groups: The first group consists of Huajiao/prickly-ash peel, Xixin /Asarum herb, warm in nature, can expel parasites and warm the internal organs; the second group consists of Huanglian/Coptis root and Huangbai /Phellodendron bark, cold in nature, can clear away stomach-heat.

Among the assistant herbs, there are Ganjiang / Dried ginger, Fuzi/ Aconite root, and Guizhi /Cinnamon twig. They can warm interior and are very useful in dispersing internal cold. Additionally Guizhi/Cinnamon twig and Xixin/ Asarum herb   facilitate the flow of yang-qi and thereby help Renshen /Ginseng and Danggui /Chinese angelica root tonify the qi and nourish blood. The vinegar used in soaking Wumei/Black plum during herbal processing is for increasing more acid to reinforce the ascaris to return to duodenum.

Dr Ke Qin in the 17th century described the effect of this formula on round-worms, said that ¡°When roundworms encounter sourness, they are calmed (De suan ze   jing); When they encounter acridity, they are spent; When encounter bitterness, they are purged¡±. So this has become the standard formula for treating Biliary ascariasis.

However, from Wumei Wan/Black Plum Pill, we have developed new recipe for treating Biliary ascariasis, for example, Dandao Qu Hui Tang/ Decoction for Removing Ascarides from Biliary Tract that is  more effective than traditional  Wumei Wan/ Black Plum Pill. It consists of Yinchen/Oriental wormwood/Herba Artemisiae Capillaris 30g, Shijunzi / Quisqualis Fruit /Fructus Quisqualis 15g, Kulianpi /Chinaberry bark/ Cortex Meliae 15g, Binglang/ Areca Seed / Semen Arecae 15g, Chuanjiao/ Pricklyash peel/Pericarpium Zanthoxyli 15g, Zhishi /Immature bitter orange/Fructus aurantii immaturus 9g, Muxiang / Aucklandia root /Radix Aucklandiae 9g, Caihu/Bupleurum root/Radix Bupleuri 15g, Huangqin/ Scutellaria root/Radix Scutellariae 9g. Decoct the above herbs for 25-30 minutes, and get the decoction for  oral use, 1-2 doses  a day, generally only  take 5-6 bags enough for the   patient with Biliary ascariasis. It is really effective, please try. See ¡°New Research in Classical Prescriptions of TCM¡±.

Indications in the original texts of Shanghan Lun

Clause 338: The illness with coldness on limbs and a feeble pulse, on the 7th or 8th day, the patient suffers continuous restlessness and irritation, and coldness on the skin. It is a case of ¡®Visceral Jue¡¯, it is not ascariasis;

If coldness on the extremities is caused by ascarides (Ascariasis Jue), the patient might vomit ascarides. The patient is quiet and feels restless occasionally. Because of deficiency cold of the spleen and intestine, ascarides come above the diaphragm, and restlessness is caused and then stops after a time. The patient will vomit and restless as soon as he eats, as ascarides sense the smell of food and crawl upward. The patient usually vomits ascarides. Wumei Wan/Black Plum Pill will be a curative for the case as well as for chronic diarrhoea.

In Chapter 19 of Jin Kui Yao Lue, it said ¡®For the patient with Hui-jue/Biliary ascariasis, Wumei Wan /Black Plum Pill could be used¡¯.

Modern clinical application

1)     We usually used Wumei Wan/ Black Plum Pill for the treatment of Biliary ascariasis, chronic dysentery, colitis, etc.

2)     Dr Liu Xue Tang reported that Wumei Wan /Black Plum is effective for diarrhoea due to bacterial imbalance after using long term antibiotics.

Modern research

1)     Get 21 of  active round-worms  by operation, and then divided into 3 groups: put 7 into the normal saline  at 37 degree centigrade , after 2 minutes, they are still active ; However , put 7 into 5% of Wumei Wan/Black Plum Decoction; and also put 7 into 30% of Wumei Wan/Black Plum  Decoction, As a results, the round-worms  in the 30% Wumei Wan/Decoction were narcose  or dead; while the activities of  those  in the 5  % of Wumei Wan/Black Plum  became less.

2)     Give Wumei Wan /Black Plum Decoction to the patients and animals with amphibolic fistula, they found that after drinking Wumei Wan /Black Plum Decoction , the  bile excretion  was  increased; pH of bile is down and  became more acid; Boden¡¯s sphincter  was relaxed.

3)     From the above tests, I think  that Wumei Wan /Black Plum Pill treated Biliary ascariasis by following 3 ways: First Wumei Wan/ Black Plum can calm and paralyse  ascarides  and make their  adhesive force to Biliary tract to become less or  lost; Second, the increased bile excretion can lash ascarides  downward and enforce ascarides to return back to the duodenum ; Third, relaxed Boden¡¯s sphincter  make ascarides  to  return back to duodenum  easily; Fourth, as ascarides have characteristics of   being  fed up acid and like alkali, after taking Wumei Wan /Black Plum, the  living condition in the Biliary tract is not convenient for ascarides any more, for example, pH is down ;and the pressure from increased bile excretion is too strong, so they have to return back to the duodenum and intestines .

4.     How to study Shanghan Lun

1)     Master the original texts of Shanghan Lun

Shanghan Lun is a very old classic which was written in old Chinese language in the Han Dynasty (about 200-219), so we should first study some archaic and ancient Chinese and then start to learn the original text of Shanghan Lun; We should understand the meaning of the texts exactly; and which clauses are most important for today¡¯s practice; what syndromes in Six Meridian Diseases are still common today; and which herbal prescriptions are really effective; and how to develop the prescriptions , etc. 

2)     Paying more attention to the formulae

The formulae in Shanghan Lun and Jin Kui Yao Lue are most valuable parts of the books. We should try our best to do more research on the recipes, including their ingredients, dosage, preparation, actions, indications, modern clinical applications as well as reading recent pharmaceutical studies. Since 1982 I have started to do some researches in the formulae in both Shanghan Lun and Jin Kui Yao Lue, and also wrote a book ¡°Research in Classical Formulae of TCM¡±, published by Yellow River Press in July, 1989.I plan to revise it again and give it a new name, ¡°New Research in Classical Prescriptions of TCM¡± to introduce some new research and developments in classical prescriptions in the world since 1999 until today, dear students, you are warm welcome to join the work.

3)     Combination between TCM and Western medicine for Studying Shanghan Lun and Jin Kui Yao Lue

Shanghan Lun is a historical TCM book, if we can use both Chinese and Western knowledge and techniques to research it, it will be more scientific and practical. We can not just repeatedly read all old texts again and again without any new development and discovery. I heard that some of our post-graduates have strong Western medicine background. I believe that it is your advance, not weakness. We should work together, step by step, to develop and modernize our TCM for the future.
